Implant Crowns

Implant crowns can be connected to dental implants using different methods, each with its own advantages and disadvantages.

Cement-Retained Crowns

 

ToothCement-retained crowns are fixed onto an implant using dental cement. A small abutment is first attached to the implant, and then the crown is cemented onto this abutment.

Advantages of Cement-Retained Crowns

Aesthetics

Allows for a more natural-looking restoration with no visible screws or connectors.

Flexibility in Design

The crown can be designed with more complex contours and aesthetics since it is not constrained by screw access holes.

Ease of Adjustment

If adjustments are needed, the crown can be removed and replaced with relative ease.

Disadvantages of Cement-Retained Crowns

Cement Remnants

Excess cement can sometimes be trapped under the crown, which may cause gum irritation or inflammation if not properly cleaned.

Removal Difficulty If the crown needs to be removed for any reason, it can be challenging to remove without damaging the crown or the underlying structure.
Potential for Loosening Over time, the cement can degrade, potentially leading to loosening of the crown.

Screw-Retained Crowns

 

ToothScrew-retained crowns are attached to the implant using a screw that is accessible through a hole in the crown. The screw secures the crown directly to the implant abutment.

Advantages of Screw-Retained Crowns

Ease of Removal

Allows for easy removal and replacement if adjustments or repairs are needed. The screw can be accessed through the crown without removing it from the mouth.

No Cement Issues

Eliminates the risk of excess cement causing complications, as there is no cement used.

Secure Fit

Provides a secure connection that minimizes the risk of crown loosening compared to cement-retained crowns.

Disadvantages of Screw-Retained Crowns

Aesthetic Compromise

The screw access hole can sometimes be visible, which may affect the aesthetic appearance of the crown, especially in the anterior region.

Screw Loosening The screw can occasionally loosen over time, which may require periodic maintenance.
Complexity The screw-retained crown design may be more complex to fabricate and fit properly.

Hybrid Crowns

 

ToothHybrid crowns combine features of both cement-retained and screw-retained crowns. They typically use a screw-retained abutment with a cemented crown on top.

Advantages of Hybrid Crowns

Aesthetic Flexibility

Allows for aesthetic adjustments with a cemented crown while maintaining the easy removal and adjustment features of a screw-retained abutment.

Stability and Function

Provides a secure fit with the option for easy removal and replacement if necessary.

Disadvantages of Hybrid Crowns

Complexity

The hybrid approach can be more complex to fabricate and may require careful planning to ensure both components work well together.

Cost Typically more expensive due to the dual-component system.
